Stable proximal femoral fractures can be managed with conventional implant with predictable results whereas unstable fractures are challenging,
and prone to complications. Dynamic hip screw is commonly used for treating unstable intertrochanteric fractures.
Recently its usage has declined due to its complications including shortening, medialisation of the distal fragment; implant cut-outs, uncontrolled lateralisation of the proximal fragment, and varus collapse. To circumvent these issues, the proximal femoral nails were introduced by th AO/ASIF group in 1998, has gained widespread popularity for treatment of trochanteric fractures in recent years. The advantage of Proximal Femur Nailing fixation is that it provides a more biomechanically stable construct by reducing the distance between hip joint and implant.
